Application - (paper or online)  Available from the Administrative Offices of the GNHTD by contacting the office at 840 Sherman Avenue, Hamden, CT 06514 in person, by mail, or by telephone at (203) 288-6282 (TDD# 288-7076).  Applications also available via the CT ADA website.  

Approval is granted pending medical documentation and an interview with transit authorities.

Once approved, username will be entered into the system and an ID will be issued.

Reservations can be booked a week in advance, same day reservations are not accepted.  

Confirmation calls are made the day before or the day of your ride.

Cancellations must be made 24 hours before the reservation.

Repeated failure to cancel may lead to suspension from the company for a period of time (varies between companies).
